"By Whom?"
"By senior officials within DS and NEA."
State Department's Accountability Review Board has issued its much-awaited report on the events in Benghazi.

The report singles no individual (s) out for criticism or recommendation for disciplinary action though it does refer to "senior officials" in two DOS bureaus (Diplomatic Security and Near Eastern Affairs) who demonstrated lack of management and coordination.
Here is the report:

“The solution requires a more serious and sustained commitment from Congress to support State Department needs,” insists the report.
That's right; blame Congress.
Here is Hillary Clinton's letter to the Senate Foreign Relations Committee.

"Blah blah blah, woof woof woof, quack quack quack."
So there it is. It is pretty much like the Justice Department Inspector General's report on Operation Fast and Furious. Just like Eric Holder, Hillary Clinton is spared, leaving her free to issue her own statement and throw a couple of "senior officials" overboard if she chooses. When it is all said and done, Hillary is now free to set her sights on her next big prize, the presidency, just as Holder awaits his chance to be appointed to the Supreme Court. The only thing that can get in Clinton's way now is that inconvenient issue of her testifying before Congress, which she is doing a pretty good job of dodging at the moment now with that concussion.
